Birthdays are the normal method of going through age transition in the Sims. If you don't have a birthday party, your Sim will be forced through an age transition at the end of the birthday. The Sims 3 game will warn you when your Sim is about to have a birthday, and you should be prepared when they are about to age or grow up so that they can have a nice party. I'm not sure that it has such a big impact on the game, but it's fun to watch a birthday party and a good way to make family friends for your Sims.
Setting up the Birthday Party
Use the phone and select throw party. Make it a birthday party, and select the time you'd like it to begin. You'll also have to choose the dress code for the party. Rich Sims might want formal attire. Then, you'll be able to select from a list of Sims that your Sim knows. Most Sims you call will show up. Make sure that everything is ready when the guests arrive. Things get chaotic once the party starts.
You should have a nice source of entertainment at the party, and arrangements so that most people can have a seat to sit and eat. If your Sim is a great cook, you'll do well to serve your own food. For larger parties, look for the buffet table in the buy mode's kitchen tab (under kitchen tables). You can serve food on this table by clicking it. Doing so costs $250, but should do well to serve food to more guests. For smaller parties with the host being a bad cook, or not having enough time, order a pizza. This is often quite good enough to please most guests. Try to have music, television and other entertainment so that guests can mingle and do various fun things at the party.
Where are Birthday Cakes in the Sims 3?
This question gets asked a lot! You can find the birthday cakes on the outdoors tab, under outdoor activities (click the football). Click the cake once it's on the counter or table, to allow your Sim to choose who gets to blow out the candles. Choose the birthday boy or girl. Every Sim at the party will come to gather round, and your Sim will blow out the candles. At that point, they will grow/age into the next life stage.
Aging Up Early
Your Sim can age up early one time during their life. Do this by buying a birthday cake and blowing out the candles and they'll get older. Use it wisely.

To use the cake, click on it (doesn't matter which sim you are using) then click on the person you want to age up. The people will then celebrate before sparkles will swirl around the birthday sim and that sim will age up.
You can only use one cake per birthday (but if there are two people having a birthday, use two cakes (just so you don't get confused))
I hope this answered your question :)
